Welcome to our online registration!

Enrollees must register for the DUI program in the county where they live, work or attend school, unless an exception has been granted.  Please have the items listed below as they will assist you in the registration process:

  • Original official lifetime driver record dated within 30 days of enrollment—Florida driving records are required for the evaluation appointment. Florida Driver records are obtained by our office at a cost of $12.00 which is included in your prepaid online enrollment fee.
    Required for Evaluation Appointment.

  • Readable Copy of Arrest Affidavit—can be obtained at the Clerk of the Circuit/County Court for Hillsborough County arrests. If you had a traffic crash, you must provide a copy of the Traffic Crash Report, which can be obtained from the arresting agency.
    Required for Evaluation Appointment.

  • Blood or Urine Test results (if available).
    Required for Evaluation Appointment.

  • Arrest Citation Copy—if available.

  • Any information from court, such as; court date, disposition, probation information, et cetera.

  • The registration fees are:

    • Level I Online:
    •    $233.00 (Includes state assesment fee and DRI)
    •   $12.00 (Florida Driving Record)
    •   $7.00 (Online Fee)
    •    $252.00 Level I Total
    • Level II Online:
    •   $363.00 (Includes state assesment fee and DRI)
    •   $12.00 (Florida Driving Record)
    •   $7.00 (Online Fee)
    • $382.00 Level II Total

Please be advised that you are pre-registering for the program. You will need to contact our office at (813) 875-6201 during regular business hours to schedule your class and evaluation appointments and complete the registration process.

After you have scheduled your class and DUI evaluation appointments, we will send you a copy of your Evaluation/Class Assignment Receipt. The Level I DUI course is for first time offenders who have never had a previous DUI arrest or attended a DUI course. If false or misleading information is given concerning previous DUI arrests, convictions or previous attendance at a DUI program, DUI Counterattack reserves the right to charge the full Level II DUI program fee in addition to any fees paid and the Level II DUI course will be required.

The Level II DUI course is for persons who have previously attended the Level I DUI course or have been convicted two or more times of an offense requiring DUI school attendance.

Payment of fees is acknowledgement and agreement to the program rules and regulations. All fees payable to DUI Counterattack, Hillsborough, Inc. are non-refundable.

Per Administrative Rule, any client who returns to the program more than ninety (90) days after the original enrollment to complete all or part of the program will have to re-enroll and complete the entire program, including payment of all program fees.
The following requirements must be completed within the 90 day period:

  • Enrollment paperwork.
  • An appropriate, mandated DUI education program.
  • An evaluation with a State certified evaluator.
  • If required to complete substance abuse treatment, proof of intake must be received within the (90) day period.

Please be advised that information regarding drug and alcohol patient records is protected by Federal Confidentiality Rules (42 CFR Part 2). The Federal rules prohibit DUI Counterattack from making any further disclosure of this information unless further disclosure is expressly permitted by the written consent of the person to whom it pertains or as otherwise permitted by 42 CFR Part 2. A general authorization for the release of medical or other information is NOT sufficient for this purpose. The Federal rules restrict any use of the information to criminally investigate or prosecute any alcohol or drug abuse patient.
