Mail-In Registration
DUI Counterattack offers
mail-in registration as
an alternative to registering in person. In order to register by mail you must
request, either the Mail-In Registration Form
and Program Rules by telephone,
or you may download these
forms from this site.
Enrollees must reside, work, or attend school in
Hillsborough County to register at DUI Counterattack, Hillsborough, Inc.,
unless an exception has been granted.
After reading the Program Rules
and completing the
Mail-In Registration Form, you must return both signed forms, along with
the items listed below and the appropriate program fee (money order or credit
card) to:

THE ITEMS LISTED BELOW MUST BE INCLUDED WITH THE SIGNED PROGRAM RULES &
THE MAIL-IN REGISTRATION FORM.
PLEASE DO NOT INCLUDE CASH.
You will not be scheduled for class or evaluation until the following items
have been received by the DUI Program:

Current picture ID (send a copy - DO NOT SEND ORIGINAL).

Official (original) lifetime driving record dated within 30 days of enrollment.
Florida driving records can be obtained at the Clerk of the
Circuit/County Court.

Arrest Affidavit. (Can be obtained at the Clerk of the
Circuit/County Court for Hillsborough County arrests.)

Arrest Citation (if available).

Blood or Urine Test Results (if applicable).
FAILURE TO PROVIDE THE ABOVE INFORMATION WILL RESULT IN A DELAY OF ENROLLMENT
UNTIL PROVIDED.
Upon receipt of the above items along with the signed
Program Rules, the
Mail-in Registration Form, and the Program Fee, your class and evaluation
appointment dates and times will be forwarded to you by certified mail.
If you enroll in or attend a Level I DUI
course by giving false or misleading information or by omission or
failing to be forthcoming with information concerning previous DUI arrests or
convictions or previous attendance at a DUI program, you will be required to
re-enrolled and complete the Level II DUI
program, including the evaluation appointment. In addition, you will be
required to pay the full fee for Level II.
